Adult Programs Forms

The Adult Programs Committee and the PUURL Program is staffed by Kathy Strawser, Lifespan Faith Development Director, reached at kstrawser@wsuuc.org. Kathy manages all the forms the program involves.

Click on the links to your left to access, fill out and e-mail forms, or for pfd. forms, to download them to fill out manually, and bring or mail them into church .

Registration: All PUURL classes request registration (see form below). Since many classes have minimum enrollments, deciding to come at the last minute could mean that a class you wanted to attend was unnecessarily cancelled because not enough people formally registered.

Evaluation of classes: All participants are asked to complete an evaluation form at the last class session. People who miss that session could download the form using the link to your left and bring or mail it in.

Class Proposals. The Adult Programs Committee welcomes ideas for classes that would add to our program. Use the class proposal on-line form to submit such ideas to the committee.

Cost is $10 for member/pledging friend; $20 for non-member, for any and all semester offerings
